Important Reminders
  • Total Contract Price (TCP) is inclusive of EVAT and Perpetual Care Fund (PCT)
  • The 20,000 Reservation Fee is valid for 30 days and is non refundable
  • Check and Cash are the modes of payment
  • Please make checks payable to "The Fort Park Estate, Inc."
  • Amortizations to be covered by Post Dated Checks (PDC). Amortizations can be deposited direct to our bank account upon arrangement. Late amortizations incur 3% surcharge per month until fully paid. The Fort Park Estate Inc. is not duty bound to follow up or remind payment.
  • Buyer's Information Sheet/Resevation Agreement (BIS/RA), Niche Map Locator, Proof of Billing (i.e. Utility Bills); and Govt issued ID's are required.
  • Vault can accommodate four (4) standard size urns (6.5"w x 11" tall)
  • All payments are non-transferable and not refundable.

​How can I avail?
There are 2 ways to avail.
New Normal - Online Transaction
  • Fill-out our online application form.
  • Upload one valid id with 3 specimen signatures
  • Choose a unit from our map (online map is under construction)
  • Wait for evaluation of your application and confirmation of unit selected
  • If approved, you will be advised to pay online thru bank transfer or deposit
  • For installment, we can send a messenger to pickup your post dated checks

Normal - Actual Visitation
  • Set an appointment with us.
  • Bring at least 1 valid id.
  • Prepare your check/s.
  • Choose your desired unit based on your preferred location during viewing.
  • Fill-out the application form and sign.
  • Sign the Contract

Pre-Need Cremation Plan @ The Heritage Park

To respond to the needs of those families who have decided to have the remains of their dearly departed cremated, The Heritage Park now offers as part of its multi-faceted service package, the use of its state-of-the-art cremation facilities.

We have two modern crematory equipment that can complete the process faster than any other crematoria in the country. A select number of family members and friends of the deceased can opt to view and wait for the whole process to be completed in the adjacent viewing rooms.
Direct Cremation Pre-Need Service
Cremation then Viewing (Regular Chapel)
Viewing then Cremation (Premier Chapel)

Funeral Home/Viewing
Pickup of the body from hospital/residence within Metro Manila
Embalming (for viewing then cremation package only), preparation for viewing

Registration of the Death Certificate

​Major Benefits of Pre-Need Plan:
  • Fully Assignable – the plan may be assigned to any deceased person other than the Planholder
  • Fully Transferable – the planholder can transfer all rights and privileges of his plan to another person as long as the plan must be in force.
Age Requirement:
  • 18-75 years old for Spot Cash
  • 18-70 years old for 5 years to Pay
SALES Process

1. Fill out a Plan Universal Form. Leave blank the Lot and Plan Data. 
2. Attach/submit a copy of two valid government-issued IDs with 4 specimen signatures.
4. Computation: Kindly let us know your preferred term of payment upon reviewing the price list 
5. You may pay at The Heritage Park Office - Kindly schedule an appointment. Bank transfer is also accepted for pre need plans payment 


​Additional Clause:Subject to availability of items and venue. In the event that any element of the package is not available at the time of the service, we shall provide a substitute if identical or superior quality

Additional Clause: This contract and its benefits may not be transferred or assigned to another person or entity within thirty (30) days from the purchase of the same or its transfer or assignment to another party. Any such transfer or assignment that does not comply with this restrictive period shall be considered void.
